On postoperative day seven, the patient experienced profuse diarrhoea, confusion, and hyponatremia, with a CT scan revealing pneumatosis intestinalis and hepatic portal venous gas. A laparotomy found no obvious cause or ischaemic bowel. She was successfully managed with intensive care, hyperbaric oxygen therapy, broad-spectrum antibiotics, and total-parenteral nutrition, leading to a good recovery.
