Summary

What Researchers Did

Chinese researchers conducted an overview of 11 systematic reviews examining HBOT for diabetic foot ulcers, using four quality assessment tools to evaluate both the quality of the reviews and the underlying evidence.

What They Found

Only one of the 11 systematic reviews was rated high quality. No high-quality evidence was found for any HBOT outcome. Moderate-quality evidence existed for 13 outcomes. HBOT has potential in ischemic diabetic foot ulcers but should not be applied routinely to all patients.

What This Means for Canadian Patients

The evidence for HBOT in diabetic foot ulcers is real but imperfect. It works best for patients with poor circulation (ischemic ulcers). Canadian patients and clinicians should expect HBOT to be recommended selectively based on wound assessment.

Canadian Relevance

Diabetic foot ulcers are an OHIP-covered indication for HBOT in Ontario. This evidence review supports selective guideline-based use rather than universal application.

Study Limitations

Meta-analysis of systematic reviews cannot correct for underlying limitations of primary studies, and the evidence base remains heterogeneous in design and patient population.
