Researchers reported two cases of severe complications following hydrogen peroxide irrigation in war wounds.
What They Found
In both reported cases, patients experienced tachypnoea, major arterial desaturation, and a precordial "mill-wheel" murmur after hydrogen peroxide injection. One patient developed hemiplegia from paradoxical arterial gas embolism, while the other suffered pulmonary oedema. Both patients recovered after hyperbaric oxygen therapy.

Canadian patients undergoing surgery, particularly for deep or closed wounds, should be aware that hydrogen peroxide irrigation can lead to severe complications. Clinicians should strictly avoid injecting hydrogen peroxide under pressure into closed or partially closed cavities to prevent adverse events like gas embolism or pulmonary oedema.
